Step 3: Forms and Checklists

Every Group Leader and Adult Sponsor at camp must have a background check. This form ensures that your adults have been properly screened. You are required to cross check every adult sponsor with the National Sex Offender Website AND run a criminal background check.
Every church group attending FUGE Camps must submit a Certificate of Insurance (COI) listing your church’s camp dates 30 days before camp to firstname.lastname@example.org.
A certificate of insurance (COI) is a document from your insurance company showing your church’s insurance terms and limits. If your church does not have one of these on file, you may need to request it from your insurance company.
MFuge Transportation Policy (MFuge only)
Group leaders attending MFuge must read and sign the MFuge Transportation Policy, agreeing to bring adequate transportation to camp.
Sponsor’s Children at Camp Agreement (If Applicable)
Parents bringing their children to camp must complete the Sponsor’s Children at Camp form during registration on opening day and abide by those guidelines.
Children at Camp Policy
We want group leaders/adults to be able to spend more time with the ones they love during their busy summer. Children too young to participate in our camp programming (completed 5th grade or younger) can be at camp with the following parameters:
Bed space and meals = full price
No bed space or meals = no charge
Children must have adult supervision at all times.
Locations with restrictions to this policy:
Mississippi College will not allow any children on campus.
Southern Seminary will not allow any children under the age of 10 to stay on campus.
Shalimar Retreat in Panama City Beach houses up to 15 people per room so children cannot stay.
To ensure the quickest registration process for you and your students, please arrive at camp with the following information and forms already completed.
Camp Participant Form
(1 original + 1 copy + copy of insurance card)
One notarized original and one copy with photocopies of insurance cards attached for each camper and sponsor.
Please bring them separated into two sets in alphabetical order by last name. The group leader will turn in the originals upon arrival to FUGE and will keep the other set of copies at all times.
Camp Health Questionnaire
Each participant will need to complete one of these forms within 24 hours of leaving for camp and turn into the Group Leader. These forms will be turned in with the Camp Participant Forms upon arrival to camp.
Group Information Form
This form provides gender and age breakdown.
These forms are required by Jenness Park:

Glorieta requires this form, however, these forms will not be collected. Group leaders will keep these in the case of a medical situation with a student where this information will be needed.
Health History Form
You will receive an email from Glorieta Camps with a link for their form.

If you want your group to participate in laser tag or ropes course during afternoon hang time, email Scott Darnell at email@example.com
Vaccination/negative COVID tests for adults are not mandatory anymore, but still recommended.
Child Protection Training
Glorieta and Texas Locations: (Austin College, Southwestern Seminary, Houston Baptist University, Texas Baptist Camp)
We must operate under guidelines set forth by The Texas Department of State Health Services, which is the licensing agent for camps. These laws require any adult who has contact with children/youth at camp to be screened for criminal and sex offender records.
Glorieta & Texas Child Protection Training
Pennsylvania Location: (Eastern University)
We must operate under guidelines set forth by The Pennsylvania Child Protective Services (http://keepkidssafe.pa.gov/resources/clearances/index.htm), for FUGE Camps held in Philadelphia. These laws require any adult who has contact with children/youth at camp to be do the following:
Pennsylvania Child Abuse History Clearance
Pennsylvania State Police Criminal Record Check
Federal Bureau of Investigations (FBI) Criminal History Background Check
Every adult sponsor coming to camp must have had a background check within the last year. One low cost option to run these background checks is found below. You do not have to use this option for your background checks, if you have another company you would prefer to use.
